1. Participation Waiver & Assumption of Risk. I give full permission for the Player to participate in all activities conducted by Lud's Soccer Academy, its owners, coaches, and associated facilities (including Clarksville High School Soccer Complex). I understand soccer is a physical, contact sport with inherent risks of injury including sprains, fractures, concussions, heat illness, or catastrophic injury, and voluntarily assume all such risks.

2. Release of Liability & Indemnity. I release, waive, and discharge Lud's Soccer Academy, Coach Lud Elizondo, Coach Truman Hensley, the Clarksville School District, and all staff, volunteers, and facility owners from any and all liabilities, claims, demands, or actions arising out of any injury, illness, loss, or property damage sustained during academy programs.

3. Emergency Medical Treatment Authorization. If I cannot be reached immediately in a medical emergency, I authorize the coaching staff to secure proper medical treatment, select licensed physicians, transport the Player to an emergency medical facility, and authorize necessary anesthesia, injections, transfusions, or surgery. I assume responsibility for all medical costs and insurance obligations.

4. No Guarantee of Athletic Outcomes. Private training and camp instruction are developmental in nature. Lud's Soccer Academy makes no guarantees regarding specific athletic placements, varsity selection, club advancement, or collegiate scholarships.

5. Photo & Video Release. I grant Lud's Soccer Academy the right to use photographs and video of the Player captured during training for marketing, social media, and website use without compensation.

6. Binding Agreement. By signing electronically below, I certify that I have read this document, fully understand its terms, and sign it freely and voluntarily.

After You Register — What Happens Next

Confirmation: You'll receive a confirmation email within 24 hours with your invoice and payment instructions.
Spot Reserved on Payment: Your player's spot is officially reserved once payment is received. Unpaid registrations may be released after 48 hours if a round fills.
Final Details (3–5 days before camp): We'll send the daily schedule, check-in location, parking instructions, and what to bring.
Day of Camp: Arrive 10 minutes early to check in at the field. Bring cleats, shin guards, water bottle, and sunscreen. Parents may stay or drop off.
Weather: Light rain — we play. Lightning or severe weather — sessions are rescheduled. We'll text/email all parents.
