6. Organizer Waiver Acknowledgement

As the organizing parent, you confirm that all listed players' parents have agreed to the academy waiver terms. Each player's parent/guardian must sign an individual waiver before the first session.

1. Participation Waiver & Assumption of Risk. All listed players' parents/guardians give full permission for participation in academy programs and acknowledge the inherent physical risks of soccer.

2. Release of Liability & Indemnity. Parents/guardians release, waive, and discharge Lud's Soccer Academy and all staff from any and all liabilities arising from injury or property damage.

3. Emergency Medical Treatment Authorization. Coaching staff is authorized to secure proper medical treatment when parents cannot be immediately reached.

4. No Guarantee of Athletic Outcomes. Private training is developmental in nature; no guarantees regarding athletic placements or scholarships.

5. Photo & Video Release. Academy may use action photos/video from training for marketing purposes.

After You Submit — What Happens Next

Coach Review (within 1 business day): We review the roster and goals and match the group with a coach.
Time Slot Confirmed: Day & time confirmed within our standard 5–8 PM window (or requested time, if available).
Each Parent Signs a Waiver: Every player's parent/guardian must sign the individual waiver before the first session — we'll send a link.
Group Size Policy: Per-player pricing assumes 2–5+ players. If fewer than 2 players show, the session is rescheduled at no charge.
Cancellation/Reschedule: 24-hour group notice required to reschedule without forfeiting the session.
What to Bring: Cleats, shin guards, ball (if you have one), water bottle.
